ITRI has established the “Procedures for Environmental, Safety, and Health (ESH) Incident/Accident Reporting, Investigation, and Follow-up” to ensure that labs and centers experiencing incidents can promptly implement reporting, investigation, and corrective actions. Each month, all labs and centers’ incidents and follow-up improvements are consolidated, including risk reassessments and root cause
analyses. Upon closure of investigation and corrective measures, general incidents are reported during ESH Working Group meetings, while major incidents are presented at the Integrated Management System Committee meetings for project reporting, experience sharing, and horizontal deployment of preventive measures.
In 2024, a total of 28 incidents occurred, including 22 occupational injury cases, broken down as follows: 11 slips/trips/falls, four cuts, two clipping and rolling injuries, two hazardous material exposures, one struck-by injury (falling object), one collision injury, and one food poisoning case. Among these, one entanglement injury and one slip-and-fall incident were classified as major accidents. In addition, 20 near-miss events were reported. Given that slip-and-fall accidents accounted for the highest frequency, cause analyses were conducted, with strengthened preventive awareness campaigns. For cases related to facility and environmental conditions, comprehensive inspections and improvements were implemented.
